Rehabilitation, Short-term, and Long-Term Care 

When you're recovering from an illness, surgery, or injury, you may need additional care to help you heal and get back to your everyday activities. Once you have returned home from the hospital, there are a few different ways to continue your rehabilitation.

Depending on the nature or extent of your injury, your treatment might be given by physical therapists, to help you regain muscle control and function. Or you may see a speech therapist. An occupational therapist can help you relearn how to go about your daily life. You might need other specialists, depending on your situation.

Home Health Care

Lakeland Homecare may send a clinician to assess home safety, medication reconciliation, and post-surgical assessment and begin physical therapy.

Outpatient Rehabilitation

You may also begin physical therapy in an outpatient clinic. This requires that you or a friend or family member drives you to appointments in a clinic setting.

Extended Care Facility

If you are unable to return home after being discharged from the hospital, your care team may suggest additional options. If you choose to stay at an extended care facility, also known as a nursing and rehabilitation center, a care manager will provide a list of available facilities in the area and help guide you through the referral process including insurance coverage options.

  • Pine Ridge: A Rehabilitation and Nursing Center Pine Ridge is Spectrum Health Lakeland’s skilled-nursing and short- and long-term care facility. They offer professional nursing services, social and rehabilitative programs, and residential care for individuals who can no longer live independently at home.

Inpatient Rehabilitation

Another alternative to returning home – your surgeon may recommend one to two weeks of acute inpatient rehabilitation to be completed at Lakeland Hospital, Watervliet. Patients in these units have 24/7 access to comprehensive therapies, short-term rehabilitation nursing staff, and medical care to assist you in regaining as much independence as possible. During your stay, you can expect to attend physical therapy sessions for several hours a day, most days of the week.
